I.      INFORMATION REQUIREMENT: Subscriber's Information that need to be checked, updated, saved and managed are included:

1.      For Vietnamese Subscribers:

-          Full Subscriber's Number (network code or area code and subscriber number);

-          Subscriber's full name;

-          Subscriber's date of birth;

-          Subscriber's ID number or Passport number.

2.      For Foreign Subscribers:

-          Full Subscriber's Number (network code or area code and subscriber number);

-          Subscriber's full name;

-          Nationality;

-          Subscriber's date of birth;

-          Passport number, place of issue.

3.      For subscribers, who are Representatives of Agencies, Organizations:

-          Full Subscriber's Number (network code or area code and subscriber number);

-          Representative's full name (as per introduction letter);

-          Representative's date of birth;

-          Representative's ID number (including Personal ID number, Military ID number, Police ID number) or  passport number;

-          Name and address of Agency, Organization (as per introduction letter).

II.      HOW TO REGISTER:

1.      For new subscribers:

a. Fill in Registration form for prepaid service:

v     Procedure Requirement : During registration of prepaid service, customers must have the following papers:

-          For Vietnamese Individual Customers:

+        Valid ID number (including Personal ID number, Military ID number, Police ID number);

+        The original is required for comparison;

+        Information on the original Personal Identification card must be clearly presented;

+        In case, Personal ID card is not available, valid passport (at least for 3 months onwards) can be a subtitute.

-          For Foreign Customers (It is compulsory for foreigners to register at agencies, where direct connection to Customers Management & Billing System is established):

+        Valid passport;

+        Original passport is required for comparison;

+        Information on Original passport must be clearly presented.

-          For Customers, who are representatives of Companies, Agents, Organizations, Foreign Representative Offices (It is compulsory for representatives to register at agencies, where direct connection to Customers management & Billing System is established):

+        Representative's personal ID card or passport (valid for 3 months) ( as per introduction letter);

+        For government-owned enterprises : legal introduction letter is needed;

+        Registration procedure for Companies is the same as that for government-owned enterprises, plus copy of Business License;

+        For Foreign companies, Organizations and Representative offices : Introduction letter, (copy and original) Investment license, license to set up representative office in Vietnam issued by Vietnamese Authorities are needed.

-          For children at age under 14 (without ID card or passport) Parents or legal authorized guardians have to act as guarantee (Information filled in registration form belong to parents or authorized guardian).

v     Fill in Registration Form:

-          Customer's declaration:

+        After customer complete registration form, dealer checks, compares if information filled in this registration form are as adequate and accurate as required documents listed in item 2.1.a;

+        Sau đó, ký xác nhận vào phiếu đăng ký.

-          Dealer fills in registration form:

+        Check adequacy and accuracy of required documents and procedure;

+        Fill in registration form for customers;

+        Customer checks and sign in registration form.

+        Registration form are made in 2 copies. One is kept by customer and the other by agency.

b.      Register to VMS's Customers Management & Billing System:

After customer and agency have signed in registration form of prepaid service, dealer is going to update customer's information into VMS's Customers Management & Billing System.

2.      For In-Use Mobifone's Subcribers:

v     Information Requirement:

If customers register to use prepaid mobile service, they must have the same documents as required for new subscribers listed in item II.1.a.

v     How to register:

v     Register through SMS:

-          Object: Vietmamese Subscriber.

-          Sample of registration message:

+        Register by Personal ID card: ho ten.ngay sinh.so Chung minh thu.

ü      Ex: nguyen van an.01012001.012136665.   

+        Register by passport: ho ten.ngay sinh.so Ho chieu.

ü      Ex.: nguyen van an.01012001.01234564.

ü      Then, send to 1414 (free of charge).

+        Notes:

ü      Id number: at least 9 digits;

ü      Passport number: including numbers and characters;

ü      Full name/Họ và tên: key in without Vietnamese tones/viết không có dấu. a space is required between surname, middle name and first name;

ü      Date of birth in format: ddmmyyyy.

v     Register through Mobifone Portal:

-          Object: All subscriber.

-          Execution Order:

+        Go to : "Register for prepaid subscriber"; The registration form is appreared on screen: Select Registration Object:

ü      Select "Individual" if registration is for Vietnamese subscriber.

ü      Select "Foreigner" if registration is for Foreign subscriber.

ü      Select "Enterprise" if registration is for representative of Agencies, Organizations.

ü      Key in full name of subscriber in box : "Name in full".

ü      Key in subscriber's date of birth in box "Date of birth".

ü      Key in ID number (including Personal ID number, Military Id number, Police Id number) or passport number in box "ID/Passport number".

ü      Key in subscriber's address in box "Address".

ü      Then, specify province or city in box "Province/City".

ü      After filling in and checking all information that need to be registered, dealer select "Send".

v     Register directly at agencies:

-          This case is applied for agencies where direct connection to VMS's Customers Management & Billing System is established.

-          Object: All subscribers.

-          Dealers follow prevailed Process of information registration for prepaid mobile subscribers.

3.      Changef in-use customer's information:

a. When customer changes registered information, the following procedure is required:

-          The subscriber has to make request by himself.

-          In case the subscriber is not able to make request by himself, authorization letter signed by subscriber certified by (i) local Authority (if subscriber is Vietnamese) or (ii) Agencies (for the other cases) is requested.

-          Customer comes to change information need to have documents and procedure required in item II.1.a.

-          Dealer checks and compares documents and procedure.

-          Subscriber gives back previous registration form for prepaid service (if available).

-          Marks "X" in box "Change information from…..hour…..minute, date…..month….year" and fill in all information that need to be changed in registration form for prepaid service.

-          Information must be the same between previous and new registration form: Subscriber's name and subscriber nubmer. Dealer check and update information as per present regulation.

a. Customer, who transfers use right of prepaid service, has to follow the following:

v     Procedure requirement: theo qui định tại mục II.1.a.

v     Make minutes of transfer use right of prepaid service:

-          Previous or New subscribers make minutes:

+        Dealer has to check if information filled by customer in minutes of transfer use right of prepaid service and in relevant documents, procedure are the same or not. When all information are exactly the same, dealer gives minutes of transfer use right of prepaid service to previous and new subscribers to fill in information.

+        Upon previous and new subscribers' completion of the minutes, dealer checks and compares if information in required documents, procedure as per item II.1.a are adequate and accurate with information in minutes filled in by previous and new subscriber.

+        Then, dealer sign in minutes.

-          Dealer makes minutes:

+        Check adequacy and accuracy of required documents and procedure.

+        Make minutes for previous and new subscriber to sign in.

+        Minutes are made in 3 copies : one for new subscriber, one for previous subscriber and one for agency.

+        Besides, previous subscriber has to:

ü      Give back old registration form of prepaid service (if available);

ü      Pay fee for transfer use right of prepaid service as per present regulation (if applied).

+        New subscriber:

ü      Pay fee for transfer use right of prepaid service as per present regulation (if applied).

b.      How to change customer's information:

v     Through SMS:

-          Object : Vietnmaese subscriber.

-          Sample of Registratrion message:

+        Register by Id card : TD.so may.ho ten.ngay sinh.so chung minh thu.

ü      Ex.: TD.0901234567.nguyen van an.01012001.012136665.

+        Register by passport: TD.so may.ho ten.ngay sinh.so Ho chieu.

ü      Ex: TD.0901234567.nguyen van an.01012001.01234564.

ü      Then, send to 1414 (free of charge).

+        Notes:

ü      Mark "TD" when change customer's information.

ü      Id number : at least 9 digits;

ü      Passport number : including numbers and characters;

ü      Full name/Họ và tên: key in without Vietnamese tones. A space is required between surname, middle name and first name;

ü      Date of birth  in formar : ddmmyyyy.

v     Through SIM with function of information registration:

-          Object : Vietnamese subscriber.

-          Execution order:

+        Subscriber uses id/passport numbers to register:

+        Notes:

ü      Turn off Vietnamese while typing message

ü      Id number : at least 9 digits;

ü      Passport number : including numbers and characters;

ü      Full name/Họ và tên: key in without Vietnamese tones/viết không có dấu. A space is required between surname, middle name and first name;

ü      Date of birth  in formar : ddmmyyyy.

v     Through Mobifone Portal:

-          Object :All subscriber.

-          Execution order:

+        Go to : "Register for prepaid subscriber"; The registration form is appreared on screen; Select Registration Object:

ü      Select "Individual" if registration is for Vietnamese subscriber.

ü      Select "Foreigner" if registration is for Foreign subscriber.

ü      Select "Enterprise" if registration is for representative of Agencies, Organizations.

ü      Key in full name of subscriber in box : "Name in full".

ü      Key in subscriber's date of birth in box "Date of birth".

ü      Key in ID number (including Personal ID number, Military Id number, Police Id number) or passport number in box "ID/Passport number".

ü      Key in subscriber's address in box "Address".

ü      Then, specify province or city in box "Province/City".

ü      Select "Update"

ü      After filling in and checking all information that need to be registered, dealer select "Send".

v     Change directly at agencies:

-          This case is applied for agencies where direct connection to VMS's Customers Management & Billing System is established.

-          Object : All subscribers.

-          Dealers follow prevailed Process of information registration for prepaid mobile subscribers.

4.      Customer, who has lost SimCard, changes new SimCard has to follow the following procedure:

-          Provide documents proving possesion right of in-use subscriber number, including:

+          Registration form of prepaid service (if available);

+          All relevant documents regulated in item II.1.a.

-          Request has to be made by Subscriber himself. In case the subscriber is not able to make request by himself, authorization letter signed by subscriber certified by (i) local Authority (if subscriber is Vietnamese) or (ii) Agencies (if subscriber is company, agency or organization) is requested.

-          Give back previous registration form of prepaid service for the old SimCard (for prepaid subcriber who activates service before 1^st January 2008). For prepaid subcriber who activates service before 1^st January 2008, dealer checks if information in documents, procedure are the same with registed information in system. If information are adequate and accurate, dealer goes to next steps.

-          Fill in registration form to stop providing and using  prepaid service (Form BM-03). Note reason of stopping using prepaid service.

-          Fill in registration form of prepaid service as per regulation for new prepaid subscriber.

-          Pay for SimCard at price as per present regulation.
